Upon completion of the online BASICS.fst® course, you must attend a proctored examination, and you must pass the test, in order to receive the course certification.

Test consists of 15 true/false and 35 multiple choice. Passing Grade is 74%.

If you fail the test on your first attempt, you must purchase a retest in order to challenge the examination again.

PHOTO IDENTIFICATION IS REQUIRED TO WRITE THE TEST. PLEASE BRING IT WITH YOU ON THE DAY OF YOUR EXAMINATION.

TO REGISTER FOR YOUR IN CLASS EXAMINATION

To register for a test proctoring session, please complete the registration form and email a copy to:  envhealth@ihis.org. You will receive an email confirmation that you must take to the examination, along with a photo ID, in order to write the test.  If the session for which you register is full, you will be notified and requested to register for another date.

Registration is required a minimum of 14-days prior to the scheduled proctoring sessio
